Step-by-Step Instructions for Completing the CDC Form

This form must be completed in English.

Transporting dogs from Europe to the United States requires strict regulations and special documents. One such document is the “Certificate of Arrival of Dog from a Low-Risk or Rabies-Free Country” provided by the CDC (US Centers for Disease Control and Prevention). The form must be completed and certified by an official government veterinarian.

Section A: NAME, ADDRESS, PHONE NUMBER, AND EMAIL OF OWNER (CONSIGNOR)

  • Name: Enter the full name of the dog owner.
  • Organization (if applicable): Enter the name of the organization if the dog belongs to an organization.
  • Address: Enter the full mailing address.
  • City: Enter the city where the owner lives.
  • Region/State: Enter the region or state.
  • Zip Code (if in the United States): Enter the zip code.
  • Country: Enter the country where the owner lives.
  • Phone Number (Include Country Code): Enter the phone number with the country code.
  • Email: Enter the owner’s email address.

Section B: NAME, ADDRESS, PHONE NUMBER, AND EMAIL OF RECIPIENT AT U.S. DESTINATION (CONSIGNEE)

  1. If the information matches Section A, check the appropriate box.
  2. If the information is different, complete the fields as in Section A for a U.S. recipient:
  • Name
  • Organization (if applicable)
  • Address (cannot be a PO Box in the US)
  • City
  • State
  • Zip Code
  • Phone Number (including country/area code)
  • Email

Section C: ANIMAL IDENTIFICATION

  1. Pet Name: Enter the dog’s name in Latin letters (you can use Google Translate to transliterate the name from Russian or Ukrainian into English).
  2. ISO Microchip Number: Enter the microchip number.
  3. ISO Microchip Implant Date (if known): Enter the date the microchip was implanted (MM/DD/YYYY format). If the date is unknown, enter the first known date the microchip was documented in medical/vaccination records.
  4. Breed: Enter the dog’s breed.
  5. Gender: Enter the dog’s gender.
  6. Date of Birth or Age (MM/DD/YYYY): Enter the dog’s date of birth or age.
  7. Color/Markings: Describe the color and markings on the dog’s body.

Section D: OPTIONAL VACCINE INFORMATION 

  1. Product Name: Enter the name of the vaccine.
  2. Manufacturer: Enter the manufacturer of the vaccine.
  3. Batch Number: Enter the batch number of the vaccine.
  4. Expiry Date (MM/DD/YYYY): Enter the expiration date of the vaccine.
  5. Vaccination Date (MM/DD/YYYY): Enter the date of the vaccination.
  6. Next Vaccination Date (MM/DD/YYYY): Enter the date of the next vaccination.

Note:

Attach foreign veterinary records or proof of payment for veterinary services performed in a low-risk country at least six months before entry into the United States for certification by an official government veterinarian.

Section E: EXAMINING VETERINARIAN CERTIFICATION STATEMENT

  • Confirm that the veterinarian is licensed to practice veterinary medicine in the country of export.
  • Confirm that the dog has an ISO-compliant microchip and that the microchip number is correct.
  • Confirm that the dog is over six months old.
  • Confirm that the dog’s age, breed, sex, and description match those in the medical records.
  • Confirm that the dog is healthy and has not been exposed to any infectious disease in the last 30 days.
  • Confirm that the dog has not been in a high-risk rabies country or rabies quarantine area and has not had a rabies case in the last 6 months.
  • Sign the form, state the date of examination, and provide all requested details (name, address, license number, etc.).

Section F: ENDORSEMENT BY OFFICIAL GOVERNMENT VETERINARIAN IN EXPORTING COUNTRY 

1. Confirm that the veterinarian performing the examination has a valid license to practice veterinary medicine.

2. Confirm that all records and documents regarding the dog’s health are correct.

3. Sign the form, indicate the date of approval, and attach the state electronic seal or wet seal.

 Note:

  • The form is valid for entry into the United States for 30 days from the date of examination without documentation of rabies vaccination.
  • The form is valid for multiple entries into the United States as long as the documented rabies vaccination is valid.
